WASHINGTON CONSUMER HEALTH DATA PRIVACY NOTICE

This Notice applies only to the extent CHOCLEMENT LLC (“we,” “us,” “our”) collects, uses, or shares “consumer health data” as defined by Washington’s My Health My Data Act (“MHMD”).
1. Categories of consumer health data we may collect, and why we collect it
Depending on how you interact with us, we may collect the following categories of consumer health data:
- Health or wellness information you choose to share with us, such as information you voluntarily include in communications with customer support (for example, information related to PMS, menstrual-cycle experiences, symptoms, or wellness goals).
- Inferences or conclusions drawn from the information you provide above, where applicable (for example, to understand and respond to your customer support request).
- Purchase/transaction information relating to our products, to the extent that information is considered “consumer health data” under MHMD (for example, where a purchase could reasonably indicate interest in health-related topics).
Purposes / how it is used: We collect and use this information to provide the Services you request and operate our business, including: fulfilling orders, providing customer support, managing subscriptions (if applicable), improving our Services, maintaining security and fraud prevention, and complying with legal obligations.
If we offer optional features in the future (such as surveys or quizzes), we will provide additional disclosures and obtain consent where required by MHMD.
2. Sources of consumer health data
We may collect consumer health data from the following sources:
- You, when you provide it directly (such as in customer support communications).
- Your interactions and transactions with us (such as purchases, returns, or subscription management), to the extent covered by MHMD.

Service providers that support the Services (for example, ecommerce platform providers or customer support tools) in connection with your interactions with us.
3. Categories of consumer health data we may share
We may share the categories of consumer health data described above only as necessary to provide the Services you request, operate our business, and as permitted by MHMD (for example, to respond to support requests, process transactions, provide shipping/fulfillment, prevent fraud, maintain security, or comply with law).
We do not sell consumer health data.
4. Categories of third parties and specific affiliates with whom we may share consumer health data
Third-party categories: We may share consumer health data with service providers/processors that help us operate our business and provide the Services, such as:
- Ecommerce platform providers (e.g., storefront/checkout providers)
- Payment processors
- Fulfillment and shipping partners
- Customer support tools and communications providers (e.g., email/SMS delivery providers)
- IT, security, fraud-prevention, and analytics providers (to the extent necessary to operate and secure the Services)
- Affiliates: CHOCLEMENT LLC does not currently share consumer health data with affiliates (we do not currently have affiliates).
5. How to exercise your MHMD rights
Under MHMD, you may have the right to:
- Confirm whether we are collecting, sharing, or selling your consumer health data, and request access to it (including a list of third parties and affiliates with whom it has been shared).
- Withdraw consent for our collection and sharing of your consumer health data (where MHMD requires consent).
- Request deletion of your consumer health data.
How to submit a request: Email us at support@girlchocolate.co with the subject line “Washington MHMD Request” and include the email address you used to interact with us (and your order number if available). We may request additional information to reasonably verify your identity before completing your request.
Appeals: If we deny your request, you may appeal by replying to our decision email and stating that you are requesting an appeal. If your appeal is denied, we will provide information on how to contact the Washington State Attorney General to submit a complaint.
